Overview

Presented at Rhythm Interventions Online 2025 US Focus, this session chaired by chaired by Devi Nair (Jonesboro, US), examines best practice in lead extraction with a focus on the risks created by abandoned leads. Anne Kroman (Medical University of South Carolina, Charleston, US) presents a complex case that illustrates how capping leads for short term convenience can increase long term complications such as infection, venous occlusion, extraction failure and tricuspid damage. The faculty discuss mechanisms of lead entrapment, the challenges of removing cut or aged leads and strategies for safer extraction using laser systems, mechanical tools and retained venous access. They highlight the importance of early planning, avoiding partial lead withdrawal, collaborating with implanting teams on lead management and ensuring access to specialist extraction services. The panel also reviews decision making in patients undergoing tricuspid interventions where coordinated planning between electrophysiology, structural teams and surgeons is vital for safe outcomes.
